Exemption from Childhood Vaccinations

PA Code Title 28 Chapter 23 Exemptions from Childhood Vaccinations and Medical Testing

A very small percentage of Pennsylvania parents are aware that vaccinations are not required for admission to public or private k-12 schools in the state.  PA-CHD is here to show parents the fine print.

For Pennsylvania residents who need to educate themselves on the law regarding public school vaccinations and medical testing in regards to filing a religious, medical or philosophical exemption please see the PA Code title and chapter concerning vaccinations and medical examinations.  A printable PDF card is provided below.  The Philadelphia School District produces its own form but in Pennsylvania a simple declaration of exerting your rights under the PA Code Title 28 Chapter 23 is sufficient.  The code is valid for exemptions to Covid19 testing which, if desired, should be added to the language of your exemption statement letter.

EXAMPLE TEXT

We, parents name(s), being the legal guardian of name of child, object to the immunization requirements as outlined by the Department of Health of the Commonwealth of Pennsylvania as defined in 28 PA. CODE CH.23 on religious grounds or on the basis of strong moral or ethical conviction similar to a religious belief as outlined in section § 23.84 Exemption from immunization.
